Existing Patient Appointments
When you arrive at the office, please sign in and advise the receptionist if there have been any changes to your address, phone number, or insurance information so that we may keep records up-to-date.
We appreciate you showing up on time for your scheduled appointment and encourage you to show up approximately ten minutes early for registration purposes. We also understand that certain circumstances may prevent you from arriving to your appointment on time. In the event that you are running late, we always appreciate a heads up so that we may accommodate others in a timely manner. If you arrive after your scheduled appointment time, you will have to be worked into the patient line-up and we cannot guarantee that you will be seen by one of the providers in a certain time frame.
Please note: if you have not seen the doctor for three or more years, you will be considered a new patient at check-in according to our office policy.
